The primary compounds thought to be responsible for the anti-inflammatory activity of boswellia are known as boswellic acids. These compounds are thought to interfere with enzymes that contribute to inflammation and pain.

There have only been a handful of studies conducted on the effects of boswellia in treating sports injuries or arthritis, but at least a few have suggested that boswellic acids may possess anti-inflammatory activity at least as potent as common over the counter medications such as ibuprofen and aspirin. In one study of patients with rheumatoid arthritis, pain and swelling was reduced following 3 months of boswellia use.
