Specification

Companies who will apply for a API-U referred must also submit:

a. copy of Deed of Establishment and its amendments if any, and SK
Ratification of Justice

b. photocopy of domicile of the company' s headquarters are still valid from the local District Office or photocopy of a lease agreement with the manager or owner tried building

c. copy of Trading License ( SIUP) or other similar business license issued by the agencies / authorities in the field of technical service trade;

d. copy of Company Registration ( TDP) ;

e. photocopy of Taxpayer Identification Number ( TIN) of the Company or an individual and
' Responsible Company;

f. recent photograph with red background of each board or
Directors of the Company 2 ( two) pieces of size 3 x 4; and

g. photocopy of identity card or passport of the Board or Directors of the Company and personal tax ID

h. The lease / contract place of business / certificate of one' s own

Business entity or contractor in the field of energy, oil and gas, minerals and other natural resource management conducting business, based on contractual agreements of cooperation with the Government of the Republic of Indonesia, must attach:

a. copy of the Cooperation Agreement with the Government or Implementing Agency established by the Government to conduct control activities in the field of energy, oil and gas, minerals and other natural resource management;

b. The original recommendation from the Government or the Executing Agency as referred to in
letter a;

c. photocopy of Taxpayer Identification Number ( TIN) , business entities or contractors;

d. recent photograph with red background of each person in charge
Cooperation Contract 2 ( two) copies of 3x4 size, and

e. photocopy of proof of identity / passport of each person in charge.

d. The lease / contract place of business / certificate of one' s own

Companies in the field of foreign investment and domestic investment that will apply for a API-P as defined in the Head of BKPM by enclosing:

a. copy of Deed of Establishment and its amendments and their respective judicial ratification decree

b. photocopy of domicile of the company' s headquarters are still in effect from
local district office or a photocopy of the rental agreement or contract business places;

c. Registration copy of Investment;

d. photocopy of business license in industrial or other similar business license issued
by Head of BKPM;

e. photocopy of Taxpayer Identification Number ( TIN) of the Company in accordance with a domicile;

f. copy of Company Registration ( TDP) ;

g. recent photograph with red background of each board or
Directors of the Company 2 ( two) pieces of size 3 x 4; and

h. photocopy of identity card or passport of the Board or Direks and Personal Tax Registration

i. Residential Permit photocopy Expatriate ( IMTA) , especially for foreign workers
signed the API.

j. The lease / contract place of business / certificate of one' s own

Companies who will apply for a API-P to the Head of Provincial Service and a copy to the Head of District / Municipality by attaching:

a. copy of Deed of Establishment and its amendments;
and SK judicial endorsement

b. photocopy of domicile of the company' s headquarters are still in effect from
local district office or a photocopy of the rental agreement of business places;

c. photocopy of business license in industrial or other similar business license issued
by agency / technical services authorized;

d. photocopy of Taxpayer Identification Number ( TIN) of the Company in accordance with a domicile;

e. copy of Company Registration ( TDP) ;

f. recent photograph with red background of each board or
Directors of the Company 2 ( two) pieces of size 3 x 4; and

g. photocopy of identity card or passport of the Board or Directors of the Company.
